We request that The Frederick Gunn School of Washington, CT, allow the participant(s) to participate in Gunn Summer Field Hockey Clinic Series. As a condition of participation, we, the undersigned, do hereby agree to the following: Release: We understand that the participants' participation in this activity can expose them to dangers both from known and unanticipated risks. Acknowledging that such risks exist, we hereby release and discharge The Frederick Gunn School, its Officers, Agents, Employees, and representatives as well as any Clinic supervisor(s) from and against any and all claims, demands, actions, judgments, and executions which the undersigned, or any other parent or guardian or any other person, firm or corporation ever had, or now has, or may have, or which the heirs, executors, administrators, or assigns of such a person, form or corporation, as the case may be, may have or claim to have against The Frederick Gunn School, its successors or assigns, and from any liability for injuries to property, real or personal, caused by, or arising out of, during, or in connection with the above-named participant(s) in this Clinic or the rendering of emergency medical procedures or treatment, if any and employees from any and all claims or liability for personal injury or property damage I/we may suffer while participating in the activity; including, but not limited to, any claim arising out of any condition of the premises at which the activity is held or the conduct of any person in connection with the preparation for, supervision of, or conduct of any activity, whether planned or unplanned.
MEDIA RELEASE: We agree that the School may use the Participant’s/Participants' name(s), photograph(s), voice recording(s), video image(s), written work, artwork, and statements, in whole or in part, as well as the achievements of the Participant(s), in School publications, advertisements, promotion or public relations materials, or commercial undertaking, including but not limited to electronic media and the School website, without restriction or compensation. In signing below, we waive and relinquish any intellectual property rights we or the Participant(s) or we may have in the use of the Participant’s/Participants' name(s), written or otherwise recorded work, or image(s), unless we or the Participant(s) provide written notice to the School revoking such permission.
Our signatures below indicate that we have read, understood, and freely signed this agreement. We further certify that we are legally competent to sign this agreement, can read and understand ENGLISH, and fully understand its terms, had sufficient time to review and understand the risks associated with Gunn Summer Field Hockey Clinic Series, understand that we give up substantial rights by signing it, and sign if freely and voluntarily without inducement. This agreement shall be construed and enforced in accordance with the laws of the State of Connecticut, and I consent to the jurisdiction of said state.
